I suspect a bit of both. Poor operational enlistment for what was supposed to be an initial all Australian recruitment for interim results before then proceeding to the way more expensive USA sites. So to avoid missing the interim milestone they make a very expensive capital raise and and double down with shareholders funds. Can only sit tight and hope that the new interim readout (Australian and USA sites) is positive because otherwise it was a massively expensive capital raise with a higher risk strategy IMO.
