You're right mate it's poor form, but relatively minor so we simply have to chalk it up as "strike one" on the governance front and move along.
Oversights like this are fairly common, this one isn't enough to trigger a red flag for me but if & when we see this type of thing happening multiple times it's certainly time to consider popping the parachute IMHO.
