No it was from Rhythm Biosciences. You need to contact Rhythm Biosciences, not link.
From there the company secretary will give you the generic form and then you specify the option price (ie: 1.20) and how many you want excised. Then I emailed that back to the secretary with the payment receipt.
I'm guessing after this post, she is now inundated with requests!
On the other hand, having a pre-filled form with your reference details already filled in for bpay/bank transfer (like what happened with the other company I hold) makes it so much easier
