This is an extract from Oct 2016 : The Perth-based miner revealed late Monday a ventilation fan at its Appin underground mine, which is part of its Illawarra Metallurgical Coal operations in New South Wales, had failed resulting in elevated gas concentrations in Appin Area 7. Consequently, the state's mining regulator issued a prohibition notice. so a fan fails and you get a prohibition notice ! puts it in perspective .
