Mystical,
As strange as it seems, would always expect some options to expire, and could easily be explained by say, someone passing away, and surviving beneficiaries (whilst grieving) not knowing or understanding, options or shares and not being aware of, or realising implications of not exercising.
Someone who knows nothing about share market and how options work (which applies to most non hotcopper members), would never realise or fathom, that something that is worth something a few weeks ago, could suddenly disappear and be worth nothing after exercise price.
Chances are they will be told later by someone diligently assisting with finalisation of estate affairs, after which they will realise, be disappointed, and wish they knew more about options and acted quicker ... although the reality is that they will be inheriting money from deceased (all a bonus), and hence loss of deceased person (who obviously would no longer be aware or capable to get upset regarding this loss.
