Hopefully easily fixed and they would have learned from the mistakes.
Fumes would be from seals and exhaust system
Recycled air would be a matter of adding a fresh air intake
Cracked coupling, not sure about that one. I would assume replacement with new, or if they are aware of the issue they would have a solution available.
As said, hopefully this translates into improvements in design in future ships.
