Did the person who permitted the message to go out, or was responsible for the systems failed to prevent it suffer any disciplinary action?

Could there have been an ulterior motive?

Please convince me there was not and that reasonable precautions have been taken to prevent this happening again. Does someone keep score on these sort of infractions when reviewing banking license?
