    I think the Government might chip in to help fund the combination of the two drugs. The interested parties ie drug company, patient advocacy groups, etc would lobby the government to subsidise the drug combination with a financial and ethical standpoints. Financial obviously being reducing the number of cardio toxic patients they need to provide care for dramatically reducing and ethically needs no explanation.
