The randomisation schedule might be 1:1 meaning half of patients receive an implant and half don’t; this is often a deterrent for patients and another reason the current recruitment timeline for the pilot trials is not a very useful guide for how the pivotal will recruit (because the in the current pilot study all patients know they get an implant which can make enrolling seem more attractive).
