    You should have received a letter from AZH which you needed to sign and return in the self-addressed envelope.
    If you've done this, then wait for the window for acceptance to expire and you'll be contacted by letter again.
    If you haven't accepted, I'd do so quickly (unless you are deliberately holding out etc).
