They would use a low dose because they want to make sure injecting something foreign into someones heart doesn't increase the likelihood of death. That's the crucial first step. Once it's proven safe, they can prove it's benefits by increasing the dose. They've now shown it's safe and even with a low dose it has benefits. They are testing on real people here, not lab rats.
