These are also governance issues. These are the duties of the Board, to oversee, formally and professionally, if the executive management are persistently under-performing:
"The board is responsible for the overall governance, management and strategic direction of the organisation and for delivering accountable corporate performance in accordance with the organisation’s goals and objectives. This responsibility is usually set out in the organisation’s constitution or in the enabling legislation under which the organisation is registered or incorporated." (Australian Institute of Company Directors)
The series of boo-boo's on releases means oversight is poor at a Board level (it often is of course, these people do two or three, collect near millions even in small caps, and treat their liability responsibilities far too casually). They should be addressing with the executive team on shareholders' behalves.
