If you bought the shares (and they were settled) by the record date of the 10th of September, then you would have been on the register by the record date and it should be fine. When did you purchase your first parcel of shares?

If you were not on the register by the record date then yes, that is likely the reason you did not receive the entitlement and acceptance form.
If you were on the register, did you provide your email details ? Have you checked computershare just in case there were issues with your email provider (there should be a copy of correspondence in your computershare investor account).
