I have paid this morning using BPay. Once you have logged into Advanced Share Registry and followed instructions as per JW post above your entitlements show up on ICG page top RHS. If you pay with BPay using your online bank system then do not need to send entitlements page to ICG. If you wish to apply for shortfall shares then add $5500.00 per 100,000 extra shares to the total you pay with Bpay.

If you paid using BPay no need to send page to ICG. Keep it for your tax or superfund records.
