IMO try have suffered some unfortunate issues, mostly unforeseeable so I would not call them mistakes. When the Guyana Gov shut down all operation after that fatality was a huge blow which had the plant shut down for 3-4 months that was quite costly. In normal circumstances the area of the fatality would be evacuated and cordoned off so an investigation could take place and the rest of the mine continue to operate but some dickhead junior minister desides to shut down all operations
