It's not that confusing IMO.
It literally says "One Time Fee" for each tier.
"The Conneqt Pulse Biometric Monitor" is 100% the device.
"Arterial Tracking and Insights" would clearly refer to the readings it takes, and I'll hazard a wild guess they have in built information in the app for those who want insights into each measurement.
Cardiologist report would be a Cardiologists assessment of the data after at least a 7 day period, with professional insights, and signed off.
I suspect the subscription model will be the Conneqt portal, used in Clinical Trials, Remote/Home Care situations where a doctor can see the results remotely without a patient coming into the practice just for a reading.
This current early access appears more driven towards the general consumer, which IMO is the most exciting market to be entering as it's so large.
