I just called the company office in Perth and was initially advised there was an application form within the 300 page prospectus. I could not find it and then called back and was told to go to the ARM website and click on the blue "Invest in Innovation" box. I did that and it took me through to register my request to subscribe for shares and ultimately gave me BPay information to enable me to make payment.

I hope this helps anyone who was in the same position as me. I was also told the prospectus was not automatically posted to shareholders because with 300 pages the printing and mailing cost was prohibitive. If a shareholder telephone in requesting a hard copy it was sent to them free of charge. I have also just received an email confirming my application and advising me of the payment details.
