This same scam I have personally seen with two clients. As an observer, many will question why it was not picked up on but from what I saw, it was far more sophisticated than it sounds on paper.
The main one I was able to witness went to the level of forging emails from managers with specific financial approval, forging of signatures, forging of purchase orders, even forging emails to make it look like documents were emailed from the Printer/Scanner on that specific managers office.
The clients were always overseas and in different time zones to stop immediate checking of facts through phone and because both of these businesses usually waited the full payment window before making payment, it was always a rush to get money through at the last minute to avoid late fees.
Feel sorry for the employees involved as all the clues would only fall into place after its been discovered.
