Spoke to the registry who advised there were no additional forms. Then spoke to my broker who (after some back and forth emails) requested I send them my initial acceptance form, and they would then tack on the additional shares on their end. Hoping all goes through okay but yep I certainly would not be trying to purchase any more shares tomorrow/Monday without an extension.
