Just did the same and got quick reply, I'm now on the list. I did say a bit about being a long term holder with significant holding, don't know if that helps/is needed or not

I had looked for a link online before, only found one that required filling in my Company name so didn't bother. Today I put in Private Investor, and the form accepted that.
