The registrar just send it off to the IRS, the only way they or you know if there's a problem is if the IRS send it back to the registrar.
Phone the registrar & ask to someone who deals with GQG. They'll be able to tell you if they received your posted form & when they forwarded it onto the IRS. If the IRS sent it back the registrar should have already contacted you & asked you to complete a new form.
You won't get any help from the registrar on how to complete the form, they are just the middlemen.
