Blame will not get the approval process back on track. What is needed is careful and professional assessment of the requirements and to think of all the possibilities of what will satisfy the TGA. Some people are not suited to 'reading the room'.
We seem to have a great product. We now need 'attention to detail' as well as a big picture understanding of the requirements. I don't think we will get a 3rd chance for a long time into the future.
