I just logged onto Advanced Share Registry to see if the forms etc were sitting there. I don't hold ICG oppies but do hold TNRO which they also manage. I have converted many other companies previous. Process is usually the same.

Request the forms from advanced share.
Follow the payment instructions and make payment.
Submit the form to the given email address. (If they send you a personal BPay option, you usually don't need to send the form back).
Wait for conversion notification (usually within 2 weeks), there will also be a company announcement, Appendix 2A.
The oppie shares should be made into a nil holding or disappear altogether in your brokerage acct and the real shares should appear.
