IMO the catalyst was the bell potter conf presentation and the side discussions there, unless you think a side projects distraction is a good thing. (Was enough to get me to sell and have a HC rant). And plenty of analyst reports about disappointment on costs.
Evidence of ineptitude, (apart from an inept plan)? if that exists, that would come after the event when it is too late to take any action!
Might be a buying opp soon.
