The accident, although unfortunate, occurred during maintenance and not
through normal processing operations.
The 3 week maintenance downtime sems like a usual teething problem in the ramp-up of such a big chemical plant. It seems foolish, however, to have a fibreglass pipe exposed to such pressure and heat. One would think a hi-grade
stainless steel pipe would be more reliable. Yet, I guess, GXY is lucky that the injuries were not more serious.
