I don't think anything went wrong with Thunderbox - original pit was optimised with a 560 AUD gold price - hedged , so when gold spiked late in its initial life it was an accountants nightmare - reserve was 840 koz and that was pretty much what it produced. Re-invented by Saracen with a way higher gold price it still does what it says on the label.
