Well whole 1.4B got offered free covid vaccine which costed a lot more than $2.15. India also donated free left over vaccines to other developing countries.
Getting a patent means that it's easy to stop them and sue in case of unauthorised manufacture and supply. There can always be a case where drugs are manufactured locally at fraction of the cost and still be very very profitable by selling them at as half the US price. I am sure most (if not all) of the current expensive immunotherapy drugs are available in India and China and there is a good market. I haven't done research in to how much big companies make from sales in these countries but common-sense wise a huge turnover even at smaller margins can also result in large profits.
