For the second, I don't it your way. I read it that improvements occured only while taking the medication, and that regression to the mean occurs only after ceasing the medication (ie the experimental arms effect decays once you stop and it regresses back to that of the control once medication ceases).
There is no problem with subjective responses from caregivers as remember, subjects themselves would be too disabled to respond. It is important that caregivers themselves note changes.
