The share registry will send you a form. The registry processes the payments. Generally payment is made by cheque. The company does not control the payment methods.

The maximum value of shares you can apply for will likely be $15,000 worth. You may apply for up to this quantity whether you own 1 share or 1 million shares on the record date.

The company can create up to around 30million new shares in this way (30% of the shares on issue). If the issue price was 25c, this would equate to a maximum of 7.5million dollars approximately.
