My non scientific understanding is methylation in general helps repair DNA damage. So the unmethylated group respond better because they are not as good at repairing the DNA. That is the cancer DNA that TMZ attacks. PI3K is upregulated in about 85% of cases these groups may over lap but they are two different things. So say you are unmethylated and PI3K upregulated then both drugs may work. The adverse may also be true. There is no one drug that target all GBM at this stage. So there is room for many drugs.
