First 3 fields don't worry about, BOL enters that I think.

Quantity of shares: Your number of ASX shares multiplied by 0.128 (e.g if you had 10000 on asx, you would calculate 1280 and enter that)
Price per share: N/A (as this isn't a trade or sale in this case, it's just a transfer of shares you will already own)

As for trade and settlement dates, I'm not sure yet, but I think for settlement you can just choose any date at least 3 days after the ASX units disappear from your account (mid december). You need to send this form to the bank of luxembourg at least 3 days before settlement. Don't rely on Link Market Services doing anything at all.

You can also email the BOL email address on the form, they replied to me with some assistance. Just try to keep simple what you are asking them as they might not be supposed to help, so we don't want to annoy them too much.
