If you have a good idea for an orphan indication e-mail it to the company. The criteria for orphan status and the application forms are easily found on both EU and FDA websites. You can also find a list of indications for which orphan status has already been granted.

Be careful, though, crowded market places, such as that for HIV drugs, don't lend themselves to orphan indications. Most have already been used, particularly, if there are salvage therapies that get a broad response, even if it is short lived.
