Obviously no one at AGL knows that condition monitoring and maintenance can prevent failures.
Now there'll be the obligatory witch hunt and expensive external consultants with university degrees who have never worked on site writing reports to tell us what we already know. Reports to gather dust on shelves.
Expensive overreactions ignoring the next risk which will blind-side them.
