The BP disaster that occurred in the Gulf of Mexico was caused by the drilling company not putting enough centralizers down to ensure that the concrete was able to pack evenly around the production pipe and so it failed.
These projects need skill and knowledge and engineering to succeed. They also used a nitrogen gas to foam the cement which added to the failure.
