Just to flesh this out a bit more:
Log in to Automic
Go to 'Your Portfolio' and then on the right hand side across from your name click on 'Documents and statements'
Select 'Other Documents' on the left hand side and the form you need as well as the $ amount you need to exercise the options should be there.
As Lupe said, BPAY or EFT or cheque (and return form to them) are the payment options.
