    I'm reading the below as people weren't doing their jobs (explains the change out of maintenance personnel). I'm guessing operations/maintenance had been happily using the incorrect spec crusher liner and/or grinding media for the fresh ore until someone realised what was going on. Operations seem to be back on track now.
