Unfortunately that's the reason we need to have double blinded controlled clinical trials. Because we can't be sure what part of the treatment actually contributed to the patient's perception of pain relief. It's dependent on a multitude of factors that need to be controlled for. And if you are postulating a physical change is responsible, then you would perhaps hope for some functional improvement
