    We have no idea who is to blame, what we do know is not enough information was provided for the first one.

    This is from WA Government. Department of Mines , Industry, Regulation and Safety ( DMIRS ).
    DMIRS have timelines they must follow by law and cannot just stall , if they seek advice for in regards to a application it creates a " Stop the Clock " event.
