Hey mate, you would need to login to your account via https://www.securitytransfer.com.au/login.cfm  , then login using your details. Then on the right, there should be a section called "investor centre", under which there is an option called "reporting options". You click on that, and that tells you how you will be provided with a "proxy form" by the company.  I have selected all of them to be via email, so just yesterday I received an email from AVZ with an attached proxy form.  Either way, the company will either email this proxy form to you or post it to your address, so I'm guessing you will receive it in the post in a few days.
