Yes, it looks like we have to use the blank form on the back two pages of the Notice of Meeting document after filling in the blank space with the shareholder's name, address and HIN exactly as it appeara on the share register.
I did notice that this form incorrectly states that "The name and address shown above is as it appears on the Company's share register." and I wondered if this risks a legal challenge should the Resolutions be lost.
