Received mine...the blank voting form on the document has no provision for name,address or HIN. I'd suggest if you wish to do your own vote, tear off the blank, fill it in and add your details, or, tear the top off the green form and send it back with your vote. If you send your vote back without HIN and other details it's likely to be not counted
