I pay attention when a fund holds almost 20% of a company but I don't listen when a fund publicly discloses holdings. Even then I only take notice when it is a stock I already consider to be undervalued as part of my Bayesian approach to investing. I find that focusing on the data helps when dealing with cognitive biases.
As for premiums in small caps v large I think this is largely in the eye of the beholder as is valuation in general although to a lesser degree with more skilled investors. I look for absolute value and it rarely exists but is more common at the small end for obvious reasons. At certain points in time more of it is around but it is folly to try and wait for such times given opportunity cost.
