The Ann says either conditions were not met OR could not be met. You've assumed it's because they couldn't be met as someone left.
More likely is that someone personal KPI that person was given has not been met. Even something as simple as say the CEO not being awarded those performance rights because the share price isn't above 20 C by the end of 2022 could be it. Etc
Senior executives do often have performance rights issued based on share price performance. If not this ok revenue performance. Or some other important milestone.
