The wording of the Company statement and speed of the regulator in certifying that it is safe to resume operations indicates the accident was due more to human factors rather than equipment failure or other engineering or geological problem. The Company is right in allowing time for the workforce to cautiously resume activities and for the investigation to identify any contributing factors. I have been involved in health & safety in mines in the past. Mines are inherently dangerous places to work and it is usually a case of minimising risk and encouraging/instilling good behaviour rather than achieving the impossible goal of a totally safe working environment. The unexpected and unforeseen unfortunately does happen in mines.
