The quarterly has given us some indication of what may have caused the dam failure. Statement below taken from the quarterly when talking about the new temporary dam:

"The facility is proposed to be lined with a
plastic liner to avoid the potential for saturation of the underlying foundation and or the embankment, which may have been a causal contributing factor behind the TSF 4 failure."

If this is the case would that be the responsibility of the dam designer?

There is still some chance of insurance compensation and/or compensation from the dam design company.

Not sure if everyone is already aware of this but I have been advised by RED5 that all directors will be subscribing to their full entitlement.
