Its a typical issue and response. I think the only time this problem will be resolved is when comany executives and directors are personally liable for these mistakes - till then with all the existing rules there is almost no incentive to bring these issues forward unless there is potential exposure to public.
Its extremely disappointing as APA has generally been a well run organisation excluding this.
