My Mum turns 90 next birthday. She has spondylosis and degenerative scoliosis of the lumbar spine. Back pain has been an ongoing issue for some time. Despite routine physio, the pain recently worsened to the point of being crippling. A CT scan ruled out anything sinister. Treatment options were limited. Health issues ruled out oral NSAIDs. Weaker opioids weren’t the answer – they make her too drowsy + cause bowel problems. She tried paracetamol, to no effect. It was finally agreed that an injection into the spine was the best option but late January is the earliest available appointment she could get with the recommended specialist.

At this point I cursed the fact that a TPM topical opioid patch wasn’t already in production, because my hunch is that it would be ideally suited in this situation. Then I thought about the TPM diclofenac gel I had at home. I realised that topical NSAIDs aren’t approved for, nor is there evidence of efficacy in conditions such as this. Also, elderly people have been found to be particularly susceptible to local site reaction from topical NSAIDs - at up to 40%, it’s a much higher rate than in the general population. Though I figured it was unlikely to make much of a dent in strong pain, I thought that the TPM gel was still worth a try.

Mum was sceptical (a word here about my Mum – she typically thinks that nothing will work and that no problem will work out!). On the first occasion she applied the gel herself and reported that it wasn’t much help. On checking, it seemed to me that she hadn’t applied the gel in the right area. She agreed to give it another go, but this time I rubbed it in for her. I was probably as surprised as her when, not long after, she reported that the pain was feeling much better.

That was a couple of weeks ago now and since then she has continued to use the TPM Voveran gel on a daily basis. While she still has complaints about her mobility she says that the pain is no longer a problem. What’s more, even though she tends to skin sensitivity problems, there has been no local site reaction.

I am now waiting with fingers crossed to see if this effect can last right through until the end of January.

ht...that's great that the TPM gel has eased the pain...maybe she forego the injection, and just use the wonder cream!
