    fentanyl is used in patch form because it is much more lipid soluble than morphine or oxycodone, both of which would take forever to cross the skin in any significant concentration. And there are other opioids that are more potent than fentanyl (carfentanil, sufentanil for example).

    i agree with @DocMcstuffins that creating subdermal depot opioids would be a bad idea. Opioids largely work at Receptors in the spinal cord and brain (although nerve endings do have opioid receptors). If you want local analgesia then creating a way for BTM to slowly release a acting local anaesthetic (ropivacaine say) which would act on local nerve endings with very little systemic effect.
