@bedger it is clear that our learned friends have never worked in caring for diabetics, or for that matter understand that their foot infections primarily relate to peripheral vascular disease. Wounds infected or otherwise don't heal well because of poor vascularization of the foot and they get more wounds because of friable skin and peripheral neuropathies. Given that they have wounds for longer and more frequently, it is almost entirely certain that if a physician has an opportunity to prevent an infection (without creating resistance) that they will prescribe prophylactic topical gels. They will also not risk an infection getting worse and potentially becoming osteomyelitis, sepsis or leading to an amputation. It would be indefensible to screw around with less effective treatments in a highly vulnerable population.
