My understanding is as follows after having spoken with a Nephrologist .

People with diabetes have a  chance of ending up on kidney dialysis which is obviously not good.

The  problem is, you do not know which diabetic patients may end up on dialysis .

If you did, the initial treatment regime would change to try and avoid ultimate dialysis. 

However, treating doctors do not /cannot adopt aggressive medication regimes on the basis that everyone will end up on dialysis. 

Promarker D gives an indication if the diabetic is likely to end up on dialysis with about an 85% level of accuracy .

This means the treating doctor can medicate accordingly with a lot more certainty .

In short, why would you not do a Promarker D test when initially treating diabetics. 

If this becomes mainstream, and it is looking increasingly likely, then PIL  is probably undervalued.
